Note from Nurse Jamie

The First Week of School Will Be HOT!!!!!!
Here are some suggestions to deal with the heat. As well, teachers will rotate students to cooling areas in the building to allow for a break from the heat.
  • Please send a water bottle to school with your child.
  • Consider sunscreen application before leaving home
  • Dress in light, breathable clothing in order to keep cool.
  • It is strongly suggested that the students do not wear Flip Flops or slip- on Crocs. The new playground has many rope climbing and scaling options which could be difficult to perform without proper fitting shoes.

Form and Fee Night

Form & Fee Night is Tuesday, August 20, from 4-6pm. If you have not already paid your school fees and lunch fees online then bring your checkbook to Irving at this time to do so. The forms went out over the summer, but some are also available on the District 97 website.

More fun than that, though, is this is when class assignment lists are posted. Look outside the school’s front door to see which teacher your child has. Note that the lists will come down that night in order to make some changes. Class lists are not posted online for privacy reasons. (Please do not share photos of a class list on Facebook or other public online setting.)

The school is open on Form & Fee night, so new families can tour around on their own and the kids MIGHT be able to peek in their new classrooms.

Eagle Extras, Irving’s after-school program, is accepting registrations starting at Form & Fee night as well. Details can be found here.

Families do not have to stay for the whole time. Just drop by and say hello while you fill out forms, ask any questions of the front office staff, and hang out with parents you haven’t seen in a while.

Announcement from Nurse Jamie

I will be available in my health office on August 20, 21, and 22nd from 4pm to 6pm for parents to drop off health office paper work, medications and action plans for the 2013-2014 school year. I would be happy to set up a private appointment with parents who need to discuss medical concerns before the start of the school year. You can email me atjstrokosch@op97.org with requests.
Reminder!! All medications need to have a physician order and all medications need to be labeled with the prescribing information printed on the label.
Actions plans for Food Allergy, Asthma, and Seizures are required 

Health Forms that need to be submitted preferably by the first day of school or by  October 15 are as follows
-Certificate of Child Health and Proof of Immunizations for all Kindergarten students and all students transferring from out of the district. A letter of Religious Exemption can be turned in for those declining vaccinations. Proof of a lead screening result from the age of 6 months to 6 years is required for Oak Park which is a high risk area for lead. If your kindergarten student has never been screened for lead, it will need to be done.
Vision Exam is required by October 15, 2013 for all Kindergarten and students transferring into the Illinois school system for the first time.
Dental Exam is required by May 15, 2014 for all Kindergarten and 2nd Grade

Please contact Nurse Jamie for any issues or questions related to the required forms. If exams cannot be scheduled until after October 15th, proof of appointment can be turned in to Nurse Jamie to maintain compliance with state requirements.
