Reminder!!  The date for turning in all required health forms to the school nurse is fast approaching!! Please turn in required Certificate of Child Health examination forms, Immunization records, Lead Screening results for children younger than  6 years of age, and Vision Exam forms by OCTOBER 15, 2012.  The intention of the school and district is to keep all children in school, however, without the State required school physical and immunization records, your child will not be allowed to attend school after 10/15/12. Dental forms are not due until May 15, 2013.  Please contact Nurse Jamie at 708-524-3089 for any questions or concerns. All forms can be faxed to 708-524-3056.
